I understand that insurance may not cover travel immunization services, including the office visit fee, cost of vaccines, medications or travel supplies. Everest Whole Health Pharmacy services are not covered by any provincial health plans. Payment is due at the time of service using credit or debit cards, no cash or cheques are accepted. I understand that I will receive documentation of all vaccines received. I am responsible for keeping the record in a safe place and up to date. Everest Whole Health Pharmacy keeps active records on file. There is a service fee for providing duplicate records for misplaced documents. Medications cannot be returned and will not be refunded. All health information gathered is kept confidential and is not shared without the clients consent to other health care professionals, under the Ontario Personal Health Information Protection Act, 2004, S.O. 2004, C. 3, Schedule A, and will be used for the purpose of providing safe, client specific, travel health advice.
