Everest Whole Health Pharmacy services are not covered by any provincial health plans. Payment is due at the time of service using credit or debit cards, no cash or cheques are accepted. I understand that I will receive documentation of all vaccines received. I am responsible for keeping the record in a safe place and up to date. Everest Whole Health Pharmacy keeps active records on file. There is a service fee for providing duplicate records for misplaced documents. Medications cannot be returned and will not be refunded. All health information gathered is kept confidential and is not shared without the clients consent to other health care professionals, under the Ontario Personal Health Information Protection Act, 2004, S.O. 2004, C. 3, Schedule A, and will be used for the purpose of providing safe, client specific, travel health advice.

Please dial the corresponding number for PPH in the buzzer system (#001) to be allowed entry into the building, or call 416-649-0464 for reaching us at the clinic in this situation.

Everest Whole Health Pharmacy does not accept, file or bill insurance. All appointments incur an office visit and/or vaccine administration fee, In addition to charges for vaccinations, medications, prescriptions, and travel supplies. The prescription fee is $25.00. Provincial Health Insurance does not cover vaccines for travel or travel health consultations. At the time of your appointment we will provide you with the needed documentation to submit to your insurance company.
We request that you please give our office a 48-hour notice if you need to reschedule your appointment. If you fail to show up and do not contact us with at least 48 hours prior notice. We will consider this to be a missed appointment and a $75.00 fee will be charged. Our scheduling number is 416-649-0464.
If you are more than 10 minutes late for an appointment it will need to be rescheduled.
